Responsibilities
- Provide comprehensive patient care within the case management framework, supporting development, implementation, revision, and reporting of the program
- Act as a liaison among the patient, family, nursing staff, physicians, and multidisciplinary team to optimize outcomes
- Serve as a consultant to the healthcare team on patient-specific items related to case management
- Coordinate a continuum of care for a defined patient population from pre-admission through post-discharge planning
- Ensure patients and families move efficiently through hospitalization and discharge processes

Requirements
- Active Registered Nurse license in California or eligibility to practice there
- Case management experience or exposure; 1 year preferred
- Strong communication and collaboration skills; ability to work with a multidisciplinary team
- BLS certification
- Ability to work 40 hours per week in a dayshift schedule
- Graduate of an approved RN program (education requirement)
